Synopsis

Pelvik ağrı, kadınların yaşam kalitesini önemli ölçüde etkiler ve karmaşık nöroanatomik ve nörofizyolojik mekanizmaları içerir. Bu bölüm, endometriozis ve vulvar ağrı gibi kronik durumlara odaklanarak bu mekanizmaları incelemektedir. Pelvik bölgenin somatik (T12-S5) ve viseral (T10-S5) sinir sistemlerinin karmaşık ağı, ağrı algısını ve modülasyonunu zorlaştırır. Pelvic pain significantly impacts women’s quality of life and involves complex neuro-anatomical and neuro-physiological mechanisms. This chapter explores these mechanisms, focusing on chronic conditions like endometriosis and vulvar pain. The pelvic region’s intricate network of somatic (T12-S5) and visceral (T10-S5) nerve systems complicates pain perception and modulation. Endometriosis, characterized by the growth of endometrial tissue outside the uterus, and vulvar pain, often linked to pelvic floor dysfunction, are examined to understand their roles in chronic pelvic pain. The chapter also addresses neurotrophins, inflammatory mediators, and the interplay between peripheral and central sensitization in pelvic pain’s pathophysiology. Effective management strategies, including medical treatments, physical therapy, and psychological support, are highlighted to provide a comprehensive approach to alleviating pelvic pain. Understanding these mechanisms is crucial for developing targeted and effective treatments to improve patient outcomes.
