Endometriozis ve Adenomyoziste IVF Yönetimi
Synopsis
Endometriozis ve adenomyozis, kadın infertilitesinin yaygın nedenlerinden biridir. Günümüzde bu hasta grubunun yaklaşık 1/4’ünde in vitro fertilizasyon ve intrasitoplazmik sperm enjeksiyonu gibi yardımcı üreme tekniklerine ihtiyaç duyulmaktadır. Sıklıkla birliktelik gösteren bu iki hastalığın kadınlarda fertilite üzerindeki etkileri iyi bilinmelidir. Hangi hastaya bekle gör, hangi hastaya fertilite tedavisi uygulanacağı hastaların yaşı, over rezervi, infertilite süresi, sistemik hastalıklar, eşlik eden diğer faktörler, hastalığın derecesi ve lokalizasyonu gibi birçok değişkene bağlıdır. Hala uygulanacak en optimal yardımcı üreme tekniği, tedavinin takibi ve embriyo transferi konusunda karar birliği yoktur. Bu yüzden endometriozis ve adenomyozisli hastalara uygulanacak fertilite tedavileri bireyselleştirilmelidir.
Endometriosis and adenomyosis are common causes of female infertility. Currently, approximately 1/4 of this patient group requires assisted reproductive techniques such as in vitro fertilisation and intracytoplasmic sperm injection. The effects of these two diseases, which often coexist, on fertility in women should be well known. Which patient should receive wait-and-see treatment and which patient should receive fertility treatment depends on many variables such as age, ovarian reserve, duration of infertility, systemic diseases, other accompanying factors, degree and localisation of the disease. There is still no consensus on the optimal assisted reproductive technique, follow-up of treatment and embryo transfer. Therefore, fertility treatments for patients with endometriosis and adenomyosis should be individualised.
