Synopsis
Adolesanlarda endometriomanın yönetimi, yetişkinlerdeki yönetimden belirgin şekilde farklılık gösterir. Adolesanlarda endometrioma tanısı, genellikle atipik semptomlar ve tanısal araçların sınırlı kullanımı nedeniyle zorlaşır. Vajinal ultrasonografi, cinsel aktif olmayan adolesanlarda uygulanamadığından, tanı sürecinde transabdominal ultrasonografi ve MRI gibi alternatif yöntemler tercih edilir. Ancak, bu yöntemler her zaman yeterli olmayabilir ve kesin tanı genellikle laparoskopik inceleme ile konur. Medikal tedavi, adolesanlarda genellikle ilk basamak olarak kullanılır ve hormonal tedaviler, semptomların kontrol altına alınmasında etkili olabilir. Cerrahi tedavi, medikal tedaviye yanıt vermeyen vakalarda veya büyük endometriomaların varlığında tercih edilir. The management of endometriomas in adolescents significantly differs from that in adults. Diagnosis in adolescents is often challenging due to atypical symptoms and limited use of diagnostic tools. Vaginal ultrasonography, which is not feasible in sexually inactive young patients, necessitates the use of alternative methods such as transabdominal ultrasound and MRI, though these may not always be sufficient. Definitive diagnosis often requires laparoscopic examination. Medical treatment is typically the first-line approach in adolescents, with hormonal therapies proving effective in symptom control. Surgical treatment is reserved for cases unresponsive to medical therapy or where large endometriomas are present. Post-treatment follow-up is essential to monitor symptom recurrence and assess the risk of relapse. Hormonal suppression therapy is generally recommended for relapse management. A multidisciplinary approach is crucial in both the diagnosis and treatment of adolescents, ensuring long-term health improvement and addressing the unique needs of this age group.
