Kök Rezorpsiyonlarına Genel Bir Bakış, Dış Servikal Kök Rezorpsiyonunun İncelenmesi
Synopsis
Odontoklastik hücre aktivitesi sonucu oluşan dişteki sert doku kaybı kök rezorpsiyonu olarak adlandırılır. Bur durum süt dişlerinde fizyolojik bir süreçken daimi dişlerde genelde patolojiktir. Kök rezorpsiyonunun en sık görülen nedeni travma olmakla birlikte lokal veya sistemik olmak üzere birçok sebebi bulunabilir. Rezorpsiyonlar iç ve dış kök rezorpsiyonları olarak ayrılır. Dış kök rezorpsiyonu, sement veya sement-dentin birleşim bölgesinde bir yıkımla başlar ve geri dönüşümsüzdür. Sıklıkla kadınlarda ve 21-30 yaşları arasında görülür. Dış kök rezorpsiyonunda dişler vital veya devital olabilir ve genelde semptomsuz olduğu için tesadüfen teşhis edilirler. İç kök rezorpsiyonu klastik hücre aktivasyonu ile kanal duvarlarındaki dentin tübülleri ve intraradiküler dentinin semente doğru ilerleyici yıkımıdır. Bu rezorpsiyon olgunlaşmamış dişlerde daha sık görülür çünkü bu dişlerin dentin tabakası ince ve dentin tübülleri geniştir. Dış servikal kök rezorpsiyonu sıklıkla suprakrestal olarak dişlerin servikalinde ve epitelyal ataçmanın apikalinde başlar. Yıkıcı ve ilerleyici yapısından dolayı sıklıkla diş yapısında ciddi bir kayba neden olur. Servikal çürük ya da iç kök rezorpsiyonu ile karıştırılıp yapılan uygunsuz tedaviler sonucu diş kaybı ile karşılaşılabilir. Genelde pulpa vitaldir, çok komplike vakalarda vitalitesi etkilenebilmektedir. Sungingival konumundan dolayı tedaviyi planlarken ve materyal seçimi yaparken iyi bir sızdırmazlığa ek olarak biyouyumlu olan, biyolojik olarak nötr ve onarımı teşvik eden, periradiküler dokuların rejenerasyonuna destek olan, doku sıvıları ile çözünmeyen materyallerin kullanılması önemlidir.
