Özet

Koroner kollateral geçişlerde malzeme seçimi, işlemin başarı şansı ve güvenliği açısından kritik öneme sahiptir. Kronik total oklüzyon (KTO) girişimlerinde kullanılan kılavuz tellerin, oklüzyonun yapısı ve kollateral kanalın özelliklerine göre dikkatle seçilmesi gerekir. Kollateral tel ile lezyonu geçmek, KTO prosedürünün en zorlu aşamalarından biridir. Bu aşamada esnek, yumuşak uçlu ve düşük profilli teller genellikle tercih edilir. İşlemin ilerleyişine bağlı olarak tel değişimi gerekebilir ve stratejik bir tel seçimi, hem işlemin başarısını hem de güvenliğini sağlar. Mikrokateter seçimi de KTO prosedürlerinde hayati bir rol oynar. Mikrokateterler, kılavuz telin doğru yönlendirilmesini ve desteklenmesini sağlayarak kollateral geçişlerde güvenli ve etkili bir işlem yapılmasına olanak tanır. Uygun mikrokateter seçimi, anterograd ve retrograd yaklaşımlarda başarı şansını artırırken, komplikasyon riskini de azaltır. The selection of materials in coronary collateral crossing is critical to the success and safety of the procedure. In chronic total occlusion (CTO) interventions, guidewires must be carefully selected based on the structure of the occlusion and the characteristics of the collateral channel. Crossing the lesion with a collateral wire is one of the most challenging steps in CTO procedures. In this stage, flexible, soft-tipped, and low-profile wires are generally preferred. Depending on the progression of the procedure, wire exchange may be necessary. A strategic selection of the wire is essential to ensure both the success and safety of the intervention. Microcatheter selection also plays a crucial role in CTO procedures. Microcatheters facilitate safe and effective collateral crossing by allowing for precise guidance and support of the guidewire. The appropriate choice of microcatheter increases the likelihood of success in both anterograde and retrograde CTO approaches, while reducing the risk of complications. Careful selection of both guidewires and microcatheters is essential for overcoming challenges during the procedure and achieving successful outcomes.
