Özet

Reprodüktif dönemde endometrioma yönetimi, hastanın semptomları, kist boyutu ve fertilite hedeflerine göre bireyselleştirilmelidir. Endometriomalar genellikle pelvik ağrı, dismenore ve infertilite gibi semptomlarla kendini gösterir. Tanı çoğunlukla transvajinal ultrasonografi (TVUS) ile konur. Küçük ve asemptomatik endometriomalar aktif izlem altında tutulabilir, bu yaklaşım özellikle fertilitenin korunması planlanan genç kadınlar için uygun bir seçenektir. Medikal tedavi, hormonal tedaviler ve nonsteroid antiinflamatuar ilaçlarla semptomların yönetilmesini hedefler; ancak bu yöntemler kistin tamamen iyileşmesini sağlamaz. Cerrahi tedavi, büyük kistlerin çıkarılması ve semptomların azaltılması için tercih edilir. Kistektomi, over fonksiyonunu koruma açısından ooforektomiye göre daha avantajlıdır, ancak rekürrens riski daha yüksektir. Cerrahi sonrası aktif izlem ve hormonal tedavi, rekürrens riskini azaltmak için önerilmektedir. Yönetim stratejileri, her hastanın bireysel durumu göz önünde bulundurularak belirlenmeli, özellikle fertilitenin korunması planlanan kadınlarda cerrahi müdahaleler dikkatle değerlendirilmelidir.

The management and treatment of endometrioma during the reproductive period should be individualized based on the patient's symptoms, cyst size, and fertility goals. Endometriomas commonly present with pelvic pain, dysmenorrhea, and infertility. Diagnosis is primarily made through transvaginal ultrasonography (TVUS). Small, asymptomatic endometriomas can be managed with active surveillance, a suitable approach particularly for young women with fertility plans. Medical treatment focuses on managing symptoms using hormonal therapies and nonsteroidal anti-inflammatory drugs; however, these methods do not completely resolve the cyst. Surgical treatment is preferred for the removal of large cysts and symptom relief. Cystectomy is more advantageous than oophorectomy in preserving ovarian function, but it carries a higher risk of recurrence. Postoperative hormonal therapy is recommended to reduce the risk of recurrence. Management strategies should be tailored to the individual patient's condition, with surgical interventions carefully evaluated, especially in women planning for future pregnancies.
